Chapel Event Proposal Form

  • This form is to propose specific events.
  • To submit a speaker name and not an event, please email  .
  • Copies of this form will be submitted to the Convocation Advisory Panel for consideration.
  • Before you submit a proposal, you must reserve the venue in which you plan on hosting your event.  Contact the University Scheduler at extension 6143.
  • Should your program require any AV equipment (use of screen/projector for powerpoint, use of speakers and microphone, etc...), please fill out an AV Setup Request Form in addition to this form.  NOTE: Please check with your sponsoring club or department prior to submitting the form to make sure they are OK paying any applicable fees.
  • Each Convocation program is intended to promote the spiritual development of students from a Christian perspective. Every program must fulfill this requirement.
  • Event Proposals must be submitted 3-4 weeks in advance of the event and must be in before October 16, 2009 for the Fall semester.
